With so many Americans affected by heart and vascular disease, the medical community has made great strides in helping patients manage conditions. And Bayhealth is no exception.
Our coordinated care teams help you to stay in control of your day-to-day symptoms for a better quality of life in the short and long term.
Our physicians start by listening to your needs. Then, they help you set a plan toward better health. Tools like MyChart allow for easy access to your medical history and test results, and send appointment reminders to keep you on track.
Because good heart health is more than just medicine and treatments, our care teams help you incorporate lifestyle changes into your daily routine. Patients can take advantage of support groups, dietitians and other resources such as Million Hearts Delaware and the American Heart Association. It’s all part of our coordinated effort to help you manage your heart conditions and encourage you along the way.
Healthy Hearts Club
This group provides a forum where patients and their care partners can learn how to live a heart-healthy lifestyle and maintain overall health. Meetings are held on the third Tuesday of each month and the topics and guest speakers will vary monthly. For more information or to register, call 1-877-453-7107.
