Community Health Needs Assessment

We care about your health. And every day we work to strengthen the health of our community, one life at a time.

We’re asking you to take a short survey to help us in our mission. Your answers will tell us how we can meet the healthcare needs of you and your family—and our entire community. Plus, how we’re currently doing in this area.

You will have the chance to enter a raffle for a $100 Visa gift card at the end of the survey as a thank you for taking it.

We’re required to do this Community Health Needs Assessment (CHNA) as a part of the Affordable Care Act.

The survey must be completed by April 30, 2022.

There are two different surveys: One for the community, including our patients and others living in central and southern Delaware. And one for stakeholders, including our physicians, schools, businesses, organizations, churches, and local and state government agencies.
